Registration
Information on how to register for this season is as follows:
NVGB registration packet includes these forms
- Players Contract (Fill out and return two copies)
- Medical Release form (Fill out and return one copy)
- Code of Conduct form (Fill out and return one copy)
- Volunteer Application (Fill out and return one copy)
- Volunteer Policy Form (Sign Form and return one copy)
- Uniform Order Form (Fill out and return one copy)
- Parent Participation & Skills Form (Fill out and return one copy)
Instructions on how to Register: To register online you will need Adobe Reader to fill out the form here online. You can download the program free of charge and install on your computer. Click on each form and fill out online. After completing each form you will need to click on "Print" to print out copies to provide to our team and for your records. Print two copies to give to the registrar of our team along with payment and one copy for your records. Then either drop them off at the designated registration locations or click on "Submit by email" to forward the registration to us. Your child will officially be on roster once a completion of this process is done along with full payment by the noted deadline. Note: you will not be able to save the form online so if you exit the form you will have to re-fill out the form.
Attach a copy of Birth Certificate and provide original Birth Certificate or other proof of age official document to the Players Contract and give to our team registrar. Fees will be as follows
January 1 - February 29 Early Fee
$95.00 first athlete - $10.00 discount for each additional sibling - Ask about our family discount rate of four or more participants
March 1st - March 21 Regular Registration
$125.00 first athlete - $10.00 discount for each additional sibling - Ask about our family discount rate of four or more participants
(Payment by check is accepted through March 7, 2009. There is a return check fee of $40. All transactions must be paid and settled by the final payment deadline of March 20, 2010. Please note that athletes are not officially included on roster or will receive uniforms until final payment is verified. No exceptions. )
Our season runs March to June 2009. Athletes between the ages of 5 to 15 are able to participate. Our fees include awards, ribbons, trophies, uniforms & Pictures for all registered participants.
Please note that all participants must provide Original Birth Certificate and a COPY of the birth certificate at registration for all new and returning athletes along with other required forms!
Sign ups will be as follows:
Location: Sylmar High School (Registration Table)
Date: Starting March 1, 2010 at 6:00 p.m.
Time: 10:00 a.m. to 12:00 p.m. Saturday and 5:30 p.m. to 7:30 p.m. Weekdays
